I am a licensed therapist with experience working with people with substance use problems. I offer a safe and efficient way to complete your evaluation and start your program in order to get your license back as soon as possible.

Nathan Ladd Evaluations: In person and online

Nathan Ladd Evaluations offers ADSAC evaluations in person and virtually. We are set up to do evaluations for anyone in the state of Oklahoma. All you have to do is sign the intake forms, pay the evaluation fee, and set up an appointment.

For online evaluations, you will be given a link to follow that will take you to your tele-health evaluation.

After having your license suspended due to driving under the influence or an incident involving alcohol/drugs, Oklahoma requires that individuals take an assessment to determine the level of substance use treatment that they need to complete in order to receive their license. Oklahoma uses ADSAC evaluators to do this. These evaluators are licensed mental health and substance use professionals. These professionals are qualified to administer evaluations in order to determine the intensity of any alcohol/drug problems and the recommended level of treatment.

What to expect during an ADSAC evaluation?

ADSAC evaluations usually last about 2 hours and have a few steps.

  1. The first is just agreeing to the evaluation and signing consent forms.

  2. The second is submitting payment ($160).

  3. The third part is completing the evaluation. The evaluation has three main components: the Defendant Questionnaire (DQ) or the Driving Risk Inventory (DRI); Triage Assessment for Addictive Disorders (TAAD); and a biopsychosocial assessment.

  4. Once each of these is complete, a level of treatment service and program(s) will be recommended.

  5. Once the individual has completed these recommendations, they receive a red stamp that they can present to get their license reinstated.
