Scheduling an Evaluation

Are you interested in scheduling an ADSAC evaluation with us? We know that this can be a stressful time and you just want to get back to driving with all of its freedoms. We strive to make the process as easy as possible for you. As such, we offer in-person and tele-health evaluations. In order to schedule an evaluation, please follow these steps:

• Download the forms from the intake tab. Fill them out and sign them, scan them into digital format, and email them to: 
  If you are unable to print and/or scan the intake forms, you may contact us to be sent a link to an online signing portal. 

• Once the forms are signed and received, you must pay the evaluation fee ($160). You may send a check to our address or you may request a payment link to be sent via text or email. (The payment link has a small convenience fee). Once payment is received you may schedule your appointment.

• A staff member will contact you to schedule an appointment during regular business hours.
